The gang all join a gym and Barney sees Rhonda, the woman he lost his virginity to, while working out. He discovers that she didn't rate his performance in bed, despite telling him she did at the time.

During the final moments of his last day, JD begins to wonder about the future after he leaves Sacred Heart, if his relationship with Elliot will survive, or if he will remain close to his best friend Turk, and even if Dr Cox will miss him when he is gone.

JD asks Dr Cox for lunch but runs into annoying former patient Jill Tracy, who unexpectedly teaches him something about responsibility. Elsewhere, Elliot and Carla discover that Todd's been lying about all his hospital `conquests'.

Jen and Greg go to great lengths to find the perfect babysitter for Lark and John becomes testy when shopping for a mattress.

While planning his birthday fiesta, JD finds a list of things he wanted to accomplish before he turned 30. When he discovers he has accomplished none of them, he enters a triathlon. Meanwhile, Elliot, in an effort to break her boyfriend out of his shell, discovers a side of him that freaks her out.

After finding out that Turk is still talking to an ex-girlfriend, Carla moves out to think things through.
