The gang all join a gym and Barney sees Rhonda, the woman he lost his virginity to, while working out. He discovers that she didn't rate his performance in bed - despite telling him she did at the time.

JD and Elliot attempt to help Turk and Carla through a rough patch in their marriage. Dr Cox challenges Dr Kelso to be a better leader by taking on a few patients on his own.

Turk gives JD and Carla the silent treatment after he learns about their kiss. Cox reluctantly turns to Elliot when an epileptic patient doesn't want to listen to him. Janitor does not feel comfortable in his new uniform.

JD locates an old list of things he wants to achieve before turning 30 and enters a triathlon to make up for it. Elliot finds a different side of her boyfriend that disturbs her.

JD intends on having lunch with Dr Cox, but is thwarted by an annoying former patient who teaches him about responsibility. Elliot and Carla find out that Todd has been exaggerating about his hospital conquests.

Monk takes a new medicine that alleviates symptoms of obsessive-compulsive disorder, but impairs his ability to solve crimes.
