Isobel arrives in town and intimidates a shocked Alaric into arranging a meeting with Elena. Elena is horrified when she finally comes face to face with her birth mother, especially when Isobel tells her that she will stop at nothing to find the mysterious invention John Gilbert has been looking for. Meanwhile, Stefan, Damon and Bonnie step in to help Elena, but are they doing the right thing?

Isobel unexpectedly returns, leaving Jenna devastated and causing both Elena and Alaric to lash out at John. Meanwhile, Bonnie, Damon and Jeremy try to find a spell to harness her ancestors' power, and a heartbroken Caroline attempts to figure out what to do about Matt. Elsewhere, Stefan and Damon realise that they have a new weapon.

Dr. Hope Martin returns to ask Maura for a favour; Jane worries about Lt. Col. Jones' spinal surgery; Tommy shows off baby T.J.; a building collapses.

Jane's relationship with Tony is not going well, and her new case is also a headache. It turns out that two of her clients are married to the same man. Tony, of all people, is representing the man.
