A rogue FBI agent takes Dr Ellen Sanders hostage after she is chosen to operate on the president, threatening to kill her and her family unless she assassinates him during the surgery.

When President Kincaid decides to use the services of another surgeon, Duncan threatens Ellen that there will be terrible consequences if she can't convince him to keep her on as his doctor.

Ellen and Brian decide they won't be able to escape until they've taken Duncan down, so Ellen sneaks away to see a friend of her father's who has previously helped her to solve severe problems in her life.

When Ellen disobeys Duncan's orders and stalls the attempt to assassinate the President during surgery, he threatens to take the life of a member of her family. Ellen begins to realise that there is no one she can trust.

Ellen has to make an agonising choice between saving her husband and protecting her children, and she finds herself forced to perform emergency surgery in her own home.

Ellen reveals to Duncan that she will not kill the President. However, she insists she will help him to get the bone marrow he needs to save his wife, as long as he gives her something in return - a promise to protect her children.
