Seventy-two-year-old David Guest is rushed to St. Vincent's Hospital after a road traffic accident; eyewitnesses say he was slumped over the wheel of his truck as it crashed into the front of oncoming traffic; the emergency team soon discover that David has a triple A ruptured aneurysm; twenty-five minutes after arriving into hospital, David is prepped for an emergency operation.

Emergency Department Specialist Registrar Rory Whelan treats David Keating who has been rushed to hospital after a house fire in Dublin; with his airway rapidly closing over, Rory must treat David quickly before his throats swells; once David is intubated the medical team discover dangerously high levels of carbon monoxide in his blood from the smoke.

Eoin O'Gorman arrives at Beaumont Hospital with a bleed on his brain; surgeons will have to bore two holes in Eoin's skull to release the pressure, a procedure that can have serious risks; Eoin's wife Margaret informs the head consultant Mr. Moshen Javadpour that if Eoin goes into cardiac arrest, he does not want to be resuscitated.

We meet father of two, John Walsh, on the morning of a crucial operation to remove extremely rare tumors from his spine; forty people around the world have been diagnosed with the type of tumors that John has, and of them John is the only remaining survivor.

The police are still dubious about Adam's role in the killings. He is finally filling in some of the missing pieces of his past including an explanation for his relationship with Dr Billon.

As Adam finally understands the link between his past and his present, he is alone with the knowledge that he must pay for what he has done.
